Information on the use of the forms

Access to the documents via the website, electronic completion

By clicking on the form corresponding to the given type of order/case, the PDF document can be opened; it requires the availability of at least Version 7.0 of Adobe Acrobat Reader on your computer. After opening the documents, data can be entered by clicking on the given data fields, and then the document can be saved to your computer. This is the recommended procedure for cases when a Customer often places certain types of orders (for instance, Cash transfer order, Securities transfer order). In these cases, it is sufficient to complete the "permanent" data fields pertaining to the Customer/Account Holder only once, and then, when placing the individual orders, to modify the order-specific data, such as the subject-matter of the order, quantity, beneficiary.

After completing and printing, as well as signing them, please, submit the forms to our Company.

Completion by conventional means

If you do not intend to use the option of electronic completion, or for any reason it is not possible (technical problems or the format of the data fields is not in line with the type of the given data), of course the forms may as well be completed manually. If you are not able to access our website, upon your request our customer services associates will provide you with the desired forms for later use. You can lodge such requests either in person or via the telephone, and the requested forms can be sent to you by mail.
